Course Overview

This subject introduces coaching and case management frameworks and tools that will prepare participants to assist in developing customized coaching/care goals and plans and activating suitable services and resources to meet client and family needs. The subject promotes knowledge and skills required to educate clients on health management, and available programmes and services to optimise client empowerment.

 

 

Objectives

 

Students will be able to:

  1. Demonstrate awareness of the importance of social support networks and community involvement on the well-being of older persons
  2. Utilize coaching and case management tools to promote the empowerment of clients in achieving optimal well-being as they age
  3. Navigate the web of formal and informal services to identify suitable interdisciplinary support for clients

Entry Requirements

Applicants applying for modular units which are drawn from existing full diploma or post-diploma programmes will have the same minimum entry requirements as the full qualifications.

Upon successful completion of the modular unit, students will be awarded a Statement of Result which can count towards the attainment of the Specialist Diploma in Gerontology (Optimal Ageing Practice).
